 All participants including staff who are less than 18 years of age and attending an event without their parent or legal guardian need to provide Geneva Point Center with a completed health form 3 weeks before your event. Below we have provided links to our American Camp Association (ACA) health forms. Please note that this form DOES require a physician’s signature and proof of a health examination within the last 24 months.
